New form as of June 2010:
IMPORTANT UPDATE FOR AGENCIES: Effective June 1, 2010, a new YRP form took effect. This form will no longer require the signature of an agency representative. Employees and volunteers can download the form from our website and complete it, or obtain a form and complete it when they attend our Customer Service locations
Discipline Review Policy
The GGHA reviews all Major Penalties and and other incidents. The policy is Listed Here. It requires that coaches or managers fill out Our Major Penalty Reprting Form to report any Major penalty that occurs in a game. The form and game sheet are forwarded to the GGHA Discipline Chair & OWHA & LLFHL.
This is a mechanism to allow us to support the OWHA, and also review penalties to determine whether the GGHA needs to act further.
Penalties will mirror the penalties given by the OWHA & LLFHL as a minimum. However we reserve the right to add additional penaties or requirements, as deemed necessary.
Getting Certificate Numbers taken in Non OWHA Courses
Many of your coaches, trainers, and other volunteers will take courses through the OMHA to be certified to be part of your teams staff. The OMHA and other "boys" associations have stop assigning your certification number at the clinic. Instead your number is only assigned once you are put on an OMHA roster sheet. However if you are only involved in girls hockey you will never be able to get your number
The OWHA has set up some forms to allow you to get your number assigned by the OWHA. All the forms are accessible from the OWHA Forms Page on the OWHA website. A direck link to each form is listed below;
Email or fax the form with proof of your clinic attendance (receipt, email from clinic and/or blank card issues at clinic). Historically the OWHA will not likely acknowledge receipt of your form. Check your mail for receipt of the card. If you have not received them within 2 weeks, re-send all the documentation. Keep re-sending regularly until you your cards. |
